Welcome to Y Be Fit! We are excited you have chosen us to help you work on your health-related goals. This page describes what we will be covering in your first appointment. For your convenience, below are links to PDF copies of the various documents that are given to you at your initial appointment. All links open a new browser window.

Informed Consent
Physician Release Form (if needed)

Y Be Fit is managed by a faculty member and run by student health coaches trained in fitness and health promotion. The program centers around three important areas of disease prevention and health promotion:

1. Assessment of an individual's present health and fitness

2. Education/prescription to guide and motivate participants toward their health goals

3. Follow-up counseling and monitoring of progress to help individuals optimize outcomes

The initial assessment (2nd Appointment) at Y Be Fit includes the following comprehensive wellness measures:

  • Blood pressure
  • Blood lipid levels measured by certified lab technicians
  • Nutrition analysis
  • Body composition (% body fat and waist/hip ratio)
  • Cardio-respiratory fitness (sub-maximal treadmill test)
  • Muscular strength and endurance
  • Flexibility and back health
  • General lifestyle (health risk appraisal)

3rd Appointment Reminders

Please complete the following tasks at least 2-3 business days prior to coming in for your 3rd visit

Health Risk Questionnaire

  • The Health Risk Questionnaire is accessed via your Y Be Fit Portal. Please take the time to complete all of the questions in one sitting. If you have any questions or concerns regarding the questionnaire, feel free to contact the Y Be Fit office

Food Prodigy Food Diary

  • Record your food intake for 3 consecutive days, including 2 weekdays and 1 weekend day. This means that the diary should be kept for either Thursday, Friday, Saturday OR Sunday, Monday, Tuesday
  • To record your food intake, access foodprodigyonline.com. Remember to click "Send E-mail" under the "Reports" tab once you have completed the 3 days of reporting your food intake

Blood Test at the BYU Student Health Center

  • Your blood cholesterol (including HDL, LDL, and triglycerides) will be measured at the BYU Student Health Center Blood Lab (click here for directions)
  • Remember to bring with you the signed form given to you at your initial appointment.You will give this to the receptionist at the main reception desk, where you will sign another consent form allowing the Student Health Center to send your results directly to Y Be Fit.
  • To ensure accuracy, YOU MUST FAST FOR AT LEAST 12-15 HOURS PRIOR TO HAVING YOUR BLOOD DRAWN (no food or drink, but water is permissible)
